How wRVU calculation works
Each CPT code carries a work RVU set by CMS in the Physician Fee Schedule. Your total is simple arithmetic: multiply each code's work RVU by how many of that exam you read, add the products together, then multiply by your contract's $/wRVU rate to get dollars.
Worked example: 3 CT abdomen/pelvis with contrast (74177, 1.77 each) + 2 two-view chest X-rays (71046, 0.21 each) = 5.73 wRVU — worth $229.20 at $40/wRVU.

How do you calculate wRVUs?
Multiply each CPT code's CMS work RVU by the number of times you performed that exam, then add the products together. To convert to dollars, multiply your total work RVUs by your contract's $/wRVU conversion rate — the worked example above shows the full math.
What is a work RVU (wRVU)?
A work RVU is the component of a CPT code's Medicare valuation that represents physician work — time, skill, and judgment — separate from the practice-expense and malpractice components. CMS publishes work RVUs in the Physician Fee Schedule, and radiology groups use them as the standard unit for measuring and paying for productivity.
How much is one wRVU worth in dollars?
There is no single national rate: the $/wRVU conversion factor radiologists are paid is set by each contract or employer, which is why this calculator lets you enter your own rate (it defaults to $40). For reference, Medicare itself pays about $33.40 per total RVU in 2026 under the fee schedule this calculator is built from — but that applies to total RVUs (work + practice expense + malpractice), not work RVUs alone, and is a payment rate, not a compensation rate.
Where do these wRVU values come from?
Every value is loaded from the CMS Physician Fee Schedule Relative Value File (RVU26C (Jul)) and re-verified each CMS quarterly release — the stamp at the top of this page shows the release and date it was built against. Values are never estimated or generated.
Why do some codes like PET/CT show no wRVU?
CMS designates PET and PET/CT codes as carrier-priced (status C): local Medicare contractors set the price instead of a single national work RVU, so no national value exists to display. This calculator lists those codes but excludes them from totals rather than inventing a number.
What is the difference between a wRVU and a total RVU?
A total RVU is the sum of three components: work (the physician's effort), practice expense (equipment, staff, overhead), and malpractice. Radiologist productivity and compensation formulas almost always use the work RVU alone, because it isolates the physician's own contribution.
